How can I decide if cosmetic eyelid surgery is for me?

Before undergoing Cosmetic Eyelid Surgery, you should have a clear understanding of how the procedure will change your appearance and what impact that change in appearance could have on your overall quality of life. You should have realistic goals for the results you expect to achieve and a willingness to follow the pre-surgical and post-operative protocols.

What is and Reason for having cosmetic eyelid surgery?

Blepharoplasty surgery removes the excess fat, muscle, and skin from both upper and lower lids. The results can be a refreshed appearance, with a younger, firmer eye area.

A Blepharoplasty is usually performed on adult men and women who have healthy facial tissue and muscles of the upper and lower eyelids and surrounding area. Treatable conditions that can be addressed by a Blepharoplasty include:

Excess fatty deposits that appear as puffiness in the upper eyelids.
Loose or sagging skin that creates folds or disturbs the natural contour of the upper eyelid, sometimes impairing vision.
Excess skin and fine wrinkles of the lower eyelid.
Bags under the eyes.
Droopiness of the lower eyelids, showing white below the iris which is the colored portion of the eye.
Droopiness of upper eyelids which interferes with the natural symmetry of the eye area.

Transconjunctival Blepharoplasty

Eyelid puffiness caused primarily by excess fat may be corrected by a procedure known as a Transconjunctival Blepharoplasty. This involves either the removal or repositioning of excess fat over the cheek bone.

After the fat is removed, the skin that was being stretched out by the fat can either retract back to its original position or wrinkle. If the excess skin wrinkles, a small procedure called a skin pinch is performed.

In the Transconjunctival Blepharoplasty, the incision is made inside the lower eyelid, and excess fatty material is removed. When sutures are used to close this kind of incision, they are invisible to the eye. They are also self-dissolving and leave no visible scar.

What is ptosis?

Ptosis is the medical term for drooping of the upper eyelids. This lowering of the upper eyelid margin may cause a reduction in the field of vision when the eyelid either partially or completely obstructs the pupil. Patients with Ptosis often have difficulty keeping their eyelids open.

There are many causes of Ptosis including age-related weakening of the muscle, congenital weakness, trauma, or sometimes neurologic disease.

As you age, the tendon that attaches the levator muscle, the major muscle that lifts the eyelid, can stretch and cause the eyelid to fall. This is the most common cause of a droopy eyelid. Ptosis may also occur following Lasik or cataract surgery due to stretching of the muscle or tendon.

Ptosis Surgery

Ptosis surgery involves tightening the levator muscle to elevate the eyelid. The goal is to elevate the eyelid enough to permit a full field of vision and to achieve symmetry with the opposite upper eyelid.

In severe Ptosis, when the levator muscle is extremely weak, a sling operation may be performed, enabling the forehead muscles to elevate the eyelid. Other types of repair may include surgery on the muscle on the inside of the lid in cases of small amounts of Ptosis. Dr. Kwitko will perform testing determine the best form of correction for you.

What To Expect After The Surgery

Immediately after the surgery has been completed, Dr. Kwitko may apply tiny sterilebandages. It is not crucial that the eyes be covered. However, an ointment to prevent dryness of the eye area may be used. A certain degree of swelling and bruising is normal. Cold compresses, as well as head elevation when lying down, will enhance healing and relieve discomfort. Dr. Kwitko will prescribe medication for discomfort.

For at least a week following your surgery, you will need to carefully clean the eye area (the eyes may feel sticky, dry, and itchy). Eye drops may be recommended.

Dr. Kwitko will also tell you about activities and environments to avoid in the weeks immediately following surgery. Permanent stitches will be removed in three to five days after surgery. Self-absorbing stitches will dissolve on their own.

Are you a good candidate for cosmetic eyelid surgery?

You are a good candidate for Cosmetic Eyelid Surgery if you are:

Generally healthy.
Free from life-threatening illness or medical conditions that can impair healing.
A non-smoker.
Free of serious eye conditions such as glaucoma, dry eye or a detached retina.
Free of such conditions as Graves Disease, Thyroid Eye Disease, cardiovascular disease, diabetes or high blood pressure and other circulatory disorders.
